WAIVER, RISK DISCLOSURE, AND MEDICAL CONSENT AGREEMENT
Riff's Workshop Blacksmithing Instruction — 230 Stony Ford Road, Middletown, NY 10940.
Assumption and Disclosure of Risk. Blacksmithing is inherently dangerous and involves serious risks including extreme heat, open flames, forges, and hot or molten metal; burns, cuts, eye injuries, crushing injuries and amputations; flying sparks, slag and debris; hand and power tools, grinders, presses and anvils; loud noise, bright light, fumes, smoke, dust and vibration; slips, trips and falls; and risk of permanent injury, disability or death.
Consent to Participation. I voluntarily grant permission for the Minor to participate in blacksmithing instruction and related activities conducted by Riff's Workshop.
Safety Compliance. The Minor will comply with all safety rules, instructions, and demonstrations. Failure to follow safety rules or to use required protective equipment may result in immediate removal of the Minor without refund.
Facility Disclaimer. Riff's Workshop operates within a facility it does not own or control and is not responsible for structural conditions, utilities or property conditions outside the scope of the instructional activities.
Medical Authorization. In the event of an injury or medical emergency I authorize Riff's Workshop to seek emergency medical treatment on behalf of the Minor if I cannot be reached. I accept full financial responsibility for any medical care provided.
No Waiver of Minor Legal Rights. Under New York law a parent or legal guardian generally may not waive a minor's right to bring a legal claim for personal injury. This Agreement is intended as a disclosure of risks, consent to participation, and medical authorization only.
Governing Law. Governed by the laws of the State of New York. Any legal action shall be brought exclusively in Orange County, New York.
