WAIVER, RISK DISCLOSURE, AND MEDICAL CONSENT AGREEMENT
MINOR PARTICIPANT
Riff’s Workshop Blacksmithing Instruction
This Agreement applies only to participants under eighteen years of age. It is executed by the minor participant’s parent or legal guardian referred to as the Guardian. The minor participant is referred to as the Minor.
This Agreement is made in consideration for the Minor’s participation in blacksmithing instruction and related activities conducted by Riff’s Workshop at a rented facility located at:

ACKNOWLEDGMENT OF MINOR STATUS AND GUARDIAN AUTHORITY
I certify that I am the parent or legal guardian of the Minor named below and that the Minor is under eighteen years of age. I have the legal authority to grant permission for the Minor to participate in the activities described in this Agreement.
ASSUMPTION AND DISCLOSURE OF RISK
I acknowledge and understand that blacksmithing is an inherently dangerous activity and involves serious risks including but not limited to the following:
Extreme heat, open flames, forges, and hot or molten metal
Burns, cuts, eye injuries, crushing injuries, and amputations
Flying sparks, slag, scale, and debris
Hand tools, power tools, grinders, presses, and anvils
Loud noise, bright light, fumes, smoke, dust, grease, and vibration
Slips, trips, falls, and impact injuries
Risk of permanent injury, disability, or death
I understand that these risks may arise from the actions or inactions of the Minor, other participants, instructors, or from the condition of tools, equipment, or the facility. I knowingly and voluntarily consent to the Minor’s participation with full awareness of these risks.
CONSENT TO PARTICIPATION
I voluntarily grant permission for the Minor to participate in blacksmithing instruction and related activities conducted by Riff’s Workshop. I understand that participation is optional and that the Minor may be removed from the activity for safety or behavioral reasons.
SAFETY COMPLIANCE AND CONDUCT
I agree that the Minor will comply with all safety rules, instructions, and demonstrations provided by instructors. I understand that failure to follow safety rules or to use required protective equipment may result in immediate removal of the Minor without refund.
FACILITY DISCLAIMER
I acknowledge that Riff’s Workshop operates within a facility that it does not own or control. I understand that Riff’s Workshop is not responsible for structural conditions, utilities, or property conditions outside the scope of the instructional activities.
MEDICAL AUTHORIZATION
In the event of an injury or medical emergency involving the Minor, I authorize Riff’s Workshop and its representatives to seek emergency medical treatment on behalf of the Minor if I cannot be reached in a timely manner. I accept full financial responsibility for any medical care provided.
NO WAIVER OF MINOR LEGAL RIGHTS
I understand that under New York law, a parent or legal guardian generally may not waive a minor’s right to bring a legal claim for personal injury. This Agreement is intended as a disclosure of risks, consent to participation, and medical authorization only. It is not intended to waive or release the Minor’s legal rights.
GOVERNING LAW AND VENUE
This Agreement shall be governed by the laws of the State of New York. Any legal action arising from this Agreement shall be brought exclusively in Orange County, New York.
SEVERABILITY
If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
ACKNOWLEDGMENT
I acknowledge that I have carefully read and fully understand this Agreement. I understand the risks involved in blacksmithing activities and I voluntarily consent to the Minor’s participation.
